Description

Sources Sought VETERANS AFFAIRS, DEPARTMENT OF is seeking services for Personal Care Home York CNH. This service is typically used to provide residential placement and personal care services to Veterans under the Health Care for Homeless Veterans (HCHV) program. The contractor will be responsible for serving up to ten (10) veterans per day for a five (5) year period. The services include transportation to healthcare appointments and meetings, and the facility should be located within a 25-mile radius from the York Community Based Outpatient Clinic in York, PA.

VAPHS is looking for a vendor in the Allegheny County area to accommodate various Veteran preferences for recovery goals within 30 miles of drive time of VAPHS. The Government intends to award a fixed-price, IDIQ contract to utilize beds at one or multiple facilities.
