This grant aims to address racial/ethnic disparities in HIV infection among Cis-gender Black women (CgBW) in the United States. CgBW account for the largest percentage of women living with HIV and the highest number of annual HIV infections among women. The grant seeks to fund organizations that can offer integrated care service models to provide HIV care and treatment, as well as other health care and essential support services, to Cis-gender Black women with HIV (CgBWH) through Community and Clinic Collaboratives (CCCs). These CCCs will consist of clinics, federally qualified health centers (FQHCs), community-based organizations (CBOs), and local and state health departments (HDs) serving CgBW. The funded organizations will be responsible for screening, navigating, and linking CgBWH to comprehensive and integrated health services, including chronic disease care, reproductive health, primary care, pediatric care, and aging-related services. The grant recipients will be expected to implement four strategies: (1) offering comprehensive and integrated models of care through CCCs, (2) providing whole person health screening, navigation, and linkage to STI and behavioral health services, (3) delivering culturally and linguistically responsive services that address stigma and medical mistrust, and (4) participating in a national learning collaborative to share best practices and lessons learned. The key outcomes of this project include increasing the number of CgBWH who initiate rapid ART, achieve viral suppression, and have their needs met for mental health and substance use disorder services, as well as other essential support services. The estimated total program funding for this grant is $7,500,000, and it is expected to result in three awards.
