The Blue Grass Army Depot (BGAD) Directorate of Emergency Services requires individuals to complete a Criminal Background Check Form for security access. This form, part of the National Crime Information Center (NCIC-III) Check, collects personal information such as name, social security number, date of birth, driver's license, and current address. For individuals born outside the United States, naturalization or passport numbers are required along with copies of certificates. Applicants must provide consent for the release of their criminal history records, authorizing BGAD to obtain information for suitability and eligibility determinations for security access. The Privacy Act Statement clarifies that the information is for official use only by BGAD and can be redisclosed only as authorized by law.
