This funding aims to maintain affordable, high-quality primary health care services for underserved populations, including those experiencing homelessness and agricultural workers, with approximately $274.8 million available for up to 71 awards. Eligible applicants include private non-profit entities and public agencies in the U.S. or its territories, with projects expected to serve at least 75% of targeted patient populations identified in their applications. Interested parties must submit their applications in two phases by September 24 and October 24, 2024, with the performance period running from April 1, 2025, to March 31, 2028.
