Description

The Centers for Disease Control - NCEH is offering a cooperative agreement grant opportunity titled "Advancing Health Equity in Asthma Control through EXHALE Strategies". This grant aims to improve the health and quality of life for people living with asthma. Asthma is a complex and prevalent chronic disease, affecting approximately 25 million Americans, including 4.7 million children and 20.3 million adults. The economic burden of asthma is significant, with costs estimated at $81.9 billion in medical expenses, missed school and workdays, and deaths. Disparities in asthma outcomes exist, particularly among non-Hispanic African American, Hispanic, and American Indian and Alaska Native populations. This grant is based on the EXHALE technical package, which includes evidence-based strategies such as education on asthma self-management, smoking cessation, home visits for trigger reduction and education, guidelines-based medical management, care coordination, and environmental policies to reduce asthma triggers. The goal of this grant is to address disparities by leveraging strategic partnerships and implementing the EXHALE strategies. Recipients will strengthen organizational infrastructure and collaborate with various sectors and community members to expand the reach and sustainability of asthma control services. The grant has an award ceiling of $725,000 and an estimated total program funding of $64,000,000. The deadline for electronically submitted applications is April 19, 2024, at 11:59 pm ET. For more information, contact Eric Morrisey at evm9@cdc.gov.
