- Special Care High: Residents with a minimum ADL dependency score of 2 or more, receiving complex clinical care or having serious medical conditions including coma, septicemia, diabetes with insulin injections, quadriplegia with higher ADL dependency, COPD with shortness of breath, etc.
- Special Care Low: Residents with a minimum ADL dependency score of 2 or more, receiving complex clinical care or having serious medical conditions including cerebral palsy, multiple sclerosis, Parkinson's disease, respiratory failure, tube feeding, ulcers, foot infections, radiation therapy, or dialysis.
- Clinically Complex: Residents receiving complex clinical care or requiring skilled nursing management for conditions like pneumonia, hemiplegia, surgical wounds, burns, chemotherapy, oxygen therapy, etc.
- Behavioral Symptoms and Cognitive Performance: Residents with a maximum ADL dependency score of 5 or less, experiencing symptoms such as difficulty in cognition, hallucinations, behavioral symptoms, rejection of care, or wandering.
- Reduced Physical Function: Residents primarily needing support with activities of daily living and general supervision, including various restorative nursing services.
