The Administration for Community Living is seeking applications to fund the National Chronic Disease Self-Management Education Resource Center through a cooperative agreement. The selected organization will deliver tiered technical assistance, manage a secure cloud-based participant database, conduct network evaluations, and lead national events to scale evidence-based self-management programs.
