Chronic Care Telehealth Centers of Excellence Program
Grant Opportunity Analysis
The Health Resources and Services Administration is seeking applications for the Chronic Care Telehealth Centers of Excellence Program to fund academic medical centers. Awardees will integrate innovative telehealth technologies into the full spectrum of services included in chronic care models, which encompasses chronic disease prevention, management, and the treatment of acute episodes resulting from long-term morbidity. Funded centers must also disseminate their findings through research publications and serve as national models for telehealth integration in chronic care services. HRSA estimates a total program funding of $19,000,000 to support approximately five awards with an award ceiling of $3,800,000.
