Advancing Strategies to Deliver and Sustain Evidence-Based Chronic Disease Self-Management Education Programs to Support Older Adults with Behavioral Health Conditions

The Administration for Community Living (ACL) is offering a funding opportunity aimed at enhancing evidence-based Chronic Disease Self-Management Education (CDSME) programs for older adults with behavioral health conditions. The initiative seeks to award up to five cooperative agreements, each ranging from $1,100,000 to $1,250,000, to eligible applicants, including private institutions of higher education, state and local governments, and non-profits, to develop and implement strategies that improve the health and well-being of older adults aged 60 and over.
