The Administration for Community Living (ACL) is offering a funding opportunity aimed at enhancing evidence-based Chronic Disease Self-Management Education (CDSME) programs for older adults with behavioral health conditions. The initiative seeks to award up to five cooperative agreements, each ranging from $1,100,000 to $1,250,000, to eligible applicants, including private institutions of higher education, state and local governments, and non-profits, to develop and implement strategies that improve the health and well-being of older adults aged 60 and over. This funding is crucial for fostering partnerships between Aging and Disability Services Networks and behavioral health providers, ultimately aiming to increase participation in these programs and replicate successful strategies nationwide.
