This document is "Attachment 3 – Past Performance List of References," a standardized template used in government solicitations for vendors to submit historical performance data. Additionally, vendors must indicate if the work was performed by a subcontractor, affiliate, or joint venture. The second page is a specialized disclosure section. It requires vendors to clarify relationships if they intend to use a subcontractor, affiliate, or joint venture for the current solicitation, or if the provided past performance references were executed under one of these third-party arrangements. For each of the three references, vendors must describe the specific functions, tasks, or third-party impacts on performance to ensure transparency in how the past work was executed and how the proposed team will collaborate on the new contract.
