SOURCES SOUGHT NOTICE - Replace Cooling System

This Statement of Work outlines the replacement and upgrade of the Main Distribution Frame (MDF) server room's precision cooling system at Kayenta Health Care Center (KHC) in Kayenta, Arizona. The existing Liebert system is at the end of its service life and lacks the capacity for planned IT expansions. The project's objective is to install two new six-ton Vertiv Liebert precision cooling systems with N+1 redundancy, integrated humidity control, energy-efficient operation, and low-ambient capability down to -20°F. The scope includes removing old units, installing new equipment, replacing piping and electrical components, integrating controls, and commissioning the new systems. All work must comply with federal, ASHRAE, DOE, GSA, EPA, and NEC standards. The contractor must be a licensed HVAC and electrical contractor with relevant experience and certified technicians. The project period is from the award date until December 31, 2026.

The Indian Health Service (IHS) provides a Buy Indian Act Indian Economic Enterprise Representation Form for offerors to self-certify their eligibility as an “Indian Economic Enterprise” (IEE) in accordance with 25 U.S.C. 47 and HHSAR 326.601. This form is used for solicitations, sources sought, and requests for information (RFIs). Offerors must meet the IEE definition at the time of offer, contract award, and throughout the contract performance period. Any change in eligibility requires immediate written notification to the Contracting Officer. While self-certification is required, Contracting Officers may request additional documentation, and awards are subject to protest regarding eligibility. Successful offerors must also be registered with the System of Award Management (SAM). The form warns against false or misleading information, citing penalties under 18 U.S.C. 1001 for false information and 31 U.S.C. 3729 to 3731 and 18 U.S.C. 287 for false claims during contract performance. The representation section requires the offeror to indicate whether they meet the IEE definition and provides fields for the name of the 51% owner, certifying signature, printed name, federally recognized tribal entity, business name, and Unique Entity ID (UEI).
