The Department of Veterans Affairs, specifically the Lebanon VA Medical Center, is requesting a single-source justification for the acquisition of three Arthrex Total Reverse Shoulder Sets. This Request for Quotation (RFQ) is an unrestricted acquisition under NAICS code 339113 (Surgical Appliance and Supplies Manufacturing) with an 800-employee size standard. The required items include Universal Reverse Consolidated Humeral Sets (AR-9501CHS), Full-Wedge Augmented MGS Instrument Sets (AR-9579S-FW), and Universal Reverse Consolidated Glenoid Sets (AR-9615CGS), each with a quantity of three.
