Sources Sought - Laboratory Quality Control Materials - Zuni Comprehensive Community Health Center (ZCCHC)

This document, Attachment A, is a Sources Sought Notice containing an estimated annual list of laboratory Quality Control (QC) materials. Issued for planning and market research purposes, the list does not commit the Government to any purchases. It outlines 19 liquid and lyophilized QC items—including Multiqual, IA Plus, ethanol/ammonia, spinal fluid, urine chemistry, immunology, bilirubin, and pediatric control products—along with their current part numbers, packaging units, and estimated annual quantities. This Department of Health & Human Services (HHS) and Indian Health Service (IHS) document is a self-certification form for the Indian Economic Enterprise (IEE) Representation under the Buy Indian Act (25 U.S.C. 47). It requires offerors responding to federal solicitations, Requests for Information (RFI), or Sources Sought notices to certify that they meet the regulatory definition of an IEE. To qualify, the enterprise must maintain its IEE status during three distinct phases: at the time of the offer, at the time of contract award, and throughout the entire duration of contract performance. Contractors are legally obligated to immediately notify the Contracting Officer in writing if they lose eligibility. Furthermore, successful offerors must be registered in the System for Award Management (SAM) and may be required to submit additional eligibility documentation upon request. The form warns that submitting false or misleading information is a federal violation punishable under 18 U.S.C. 1001, with false claims during performance subject to penalties under the False Claims Act.
