A key component is the project description, where contractors explain the complexity and relevance of their past work to the current submission. The client then completes sections on their role, the completion date, and signs the questionnaire. The PPQ includes a comprehensive rating system with adjective ratings (Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, Not Applicable) and detailed definitions for evaluating various aspects of performance. These aspects cover quality (technical data, standards, problem resolution, quality control), schedule/timeliness, customer satisfaction (end-user satisfaction, cooperativeness, overall satisfaction), management/personnel/labor (on-site management, workforce, government property control, expertise, small business utilization, adaptability, overall management), cost/financial management (meeting terms, innovative alternatives, invoicing, accounting system adequacy, terminations, financial problems), and safety/security. Clients are encouraged to submit the completed questionnaire directly to the offeror for inclusion in their proposal, though direct submission to the VA is also an option. The government reserves the right to verify all information provided. The document concludes with a section for additional remarks and a narrative addressing strengths, weaknesses, or deficiencies to aid in performance risk evaluation.
