AN41--Dry Ice Delivery Services - VA Greater Los Angeles Healthcare System

This Statement of Work outlines the requirements for a contractor to deliver dry ice to the VA Greater Los Angeles Healthcare System, Department of Medical Research, in support of ongoing research studies. The scope includes weekly dry ice deliveries to the West LA and Sepulveda campuses, along with the rental of three lockable dry ice bins. Emergency deliveries must be fulfilled within six hours. The contract is a Firm-Fixed Price agreement for an initial 12-month period with four optional one-year extensions, totaling 60 months, excluding weekends and federal holidays, though emergency orders may occur on these days. The contractor is responsible for timely deliveries, immediate issue resolution, advance communication of lead times, and submitting invoices with proof of delivery. The VA will certify approved invoices for contracted services and rentals. Security Accreditation Package is not required.
