Non Clinical Case Management

The Department of the Army’s Vermont Army National Guard, through the W7N4 USPFO Activity VT ARNG, is seeking a Service-Disabled Veteran-Owned Small Business to provide non-clinical case management services for VTARNG service members. The work includes coordinating care, managing service and resource utilization, and assisting patients within, between, and outside VTARNG facilities to support fitness, personal wellness, readiness, and deployability. Performance is expected at Green Mountain Armory in Colchester, Vermont during the normal duty day, typically 0600 to 1700 Monday through Friday, and the requirement is being procured as a firm-fixed-price effort under NAICS 541611. The solicitation is anticipated to post on SAM.gov on or about 26 April 2026, contractors must be registered in SAM, and the period of performance includes a four-month base period plus two option periods running through 11 July 2028.
